Manually record a medical visit with doctor, diagnosis, and clinical notes

Overview

The Log a Visit feature allows you to manually record medical visits that were not automatically synced from a SehaFile-connected clinic. This is essential for visits to private doctors, pharmacies, or clinics that do not yet use SehaFile. By logging these visits, you maintain a complete health history in one place, making it easier for future doctors to understand your medical journey.

Steps to Log a Visit

  1. Open the Visit Form -- Navigate to the "Visits" tab in the bottom navigation, then tap the "+" floating button to open the new visit form.
  2. Doctor Name (Required) -- Enter the name of the doctor you visited in the "Doctor Name" field. This is the only required name field.
  3. Clinic Name -- Enter the clinic, hospital, or practice name where the visit took place. This helps identify the location for future reference.
  4. Select Specialty -- Choose the medical specialty from the dropdown menu. Options include: General Medicine, Internal Medicine, Pediatrics, OB/GYN, Orthopedics, Surgery, Ophthalmology, ENT, Dermatology, Urology, Cardiology, Neurology, Pulmonology, Dentistry, Psychiatry, and Other.
  5. Visit Date (Required) -- Select the date of the visit using the date picker. It defaults to today's date but can be set to any past date.
  6. Chief Complaint (Required) -- Describe why you visited the doctor in the complaint text area. Be specific -- for example, "persistent cough for 2 weeks" rather than just "cough".
  7. Diagnosis -- If the doctor provided a diagnosis, enter it here. Example: "Acute bronchitis".
  8. Notes -- Add any additional notes such as doctor's recommendations, follow-up instructions, or observations.
  9. Tap "Save Visit" -- The green "Save Visit" button submits the form. While saving, the button shows "Saving..." and is disabled to prevent duplicates.

Tip: Log visits as soon as possible after they happen while details are fresh in your mind. Include the doctor's recommendations in the notes field so you can refer back to them later.

Tip: After logging a visit, you can upload related documents (prescriptions, lab orders) from the Documents section and link them to this visit for a complete record.

Editing a Logged Visit

  1. Open Visit Details -- From the Visits timeline, tap the visit card you want to edit.
  2. Tap "Edit" -- On the visit detail page, tap the edit button to modify any fields.
  3. Save Changes -- Tap "Save" to update the visit record.

FAQ

Can I log a visit from many years ago?

Yes. The date picker allows you to select any past date. This is useful for building a complete historical record of major medical events.

Can I delete a manually logged visit?

Yes. Open the visit detail page and tap "Delete." A confirmation dialog will appear. Deleted visits cannot be recovered.

What is the difference between a logged visit and a synced visit?

Logged visits are created by you and contain the information you provide. Synced visits come from clinics and contain the doctor's clinical notes, prescriptions, and lab orders. Synced visits typically have more detailed medical information.
