Create consent form templates, collect patient signatures, and manage revocations
Overview
Consent forms in SehaFile allow you to create standardized consent documents that patients sign digitally before procedures, surgeries, or any clinical intervention requiring informed consent. The system supports multiple consent types (general, procedure, anesthesia, surgery, research, photography), optional witness requirements, and full revocation tracking for legal compliance.
Creating a Consent Form Template
- Navigate to Settings > Consent Forms — Click Settings and select the Consent Forms tab.
- Click "New Consent Template" — The template editor opens.
- Fill in template details:
- Name (Arabic) — e.g., "موافقة على إجراء جراحي"
- Name (English) — e.g., "Surgical Procedure Consent"
- Type — Select: general, procedure, anesthesia, surgery, research, photography
- Body (Arabic) — Full consent text in Arabic, explaining the procedure, risks, alternatives, and patient rights
- Body (English) — English version of the consent text
- Requires witness — Toggle if a witness signature is needed (recommended for surgical consents)
- Save template — Click Save. The template is now available for patient signing.
Collecting a Patient Signature
- During a visit or pre-procedure, click "Request Consent" from the patient's record.
- Select the appropriate consent template.
- The patient reads the consent text on the screen or printed copy.
- The patient signs using the SignatureCanvas component (touch or mouse drawing).
- If witness is required, the witness also signs.
- Click "Submit Consent" — timestamp and signature data are recorded.

Frequently Asked Questions
Can a patient revoke consent after signing?
Yes. Navigate to the patient's consent history, select the consent, and click "Revoke". You must enter a revoke_reason. The system records revoked_at timestamp and the original consent remains in the audit trail.
Are consent forms legally valid as digital signatures?
Digital signatures in SehaFile are timestamped and associated with a verified patient identity (national ID). While Egyptian law is evolving on digital signatures, maintaining these records provides strong evidence of informed consent.
Can I print a signed consent form?
Yes. Each signed consent has a "Print" button that generates a PDF with the consent text, patient signature image, witness signature (if applicable), and timestamp.
